Thank you for taking the time to complete your child’s NHS health form. The information that you share through this health form provides key health information about your child to Leicestershire Partnership NHS Trust Healthy Together Public Health Nurses (School Nurses), allowing them to support your child’s transition to school. Your child will be weighed and measured as part of the National Childhood Measuring Programme (NCMP) in the spring term of the reception year. You will be contacted with the results.
The recommendation is that your child has 6-8 glasses of water every day and eats a balanced diet that includes a maximum of 2 healthy snacks containing less than 100 calories.

Registering with a dentist is free for children and young people. It’s really important that your child has regular check-ups with a dentist.
It is free for your child to have an eye test. It's essential that you book an eye test when your child starts school, if they haven't had a test within the last year. Having an eye test can detect problems early.
Your child will have their hearing checked in school by our audiology team within the first two years of school.
All children are different, however it is recommended that at the age of 5 years they should have 11 hours of sleep every night. This helps them to be happy and alert for the school day.
